Our permanent care focuses on enabling you to continue living your best life through encouraging you to participate when it comes to your ongoing care and daily experiences. We do this through regular assessment and care planning, reviews and case management stages that promote your independence and belonging in your community. We support you to live productively, that your health and wellbeing is a focus, that you feel daily purpose and that you stay connected.

This is done through regular re-assessment, care planning, reviews, and case management stages that promote your independence and connection with your community. We will support you to live productively, be healthy, feel you have purpose and stay connected to your community.
Allied Health Services
- We enjoy regular visits from valued Allied Health professionals delivering services including Physiotherapy, Podiatry, Speech Pathology, Audiology, Dentistry, Optometry, Occupational Therapy and more.
- If you have a preferred provider you are already seeing, we are happy to help you organise to keep seeing them. We want you to feel as comfortable as possible in your new home.
- Each of our homes liaise with other services as needed, and we do this in collaboration with you and your representatives. Whenever possible, these services can be provided in our homes, though private transport can easily be arranged for external appointments when it is needed.
Emotional Support
- And when life gets you a bit down (it happens to us all!), or you are needing some extra support, we have social workers who are only too happy to help you work through what you are feeling.
- This is a free service that can include counselling, loss and grief, transition to aged care, public trustee, guardianship orders, communication difficulties, understanding of behaviours, liaising between health professionals and families, family conflict, drug and alcohol issues, case conferencing, mediation, financial assistance, links to community groups/services, advocacy and support.
- Our team also have access to counsellors and psychologists through our Infinite Thrive program so they are able to be their best selves for you at all times.
